Omega supports frail-elderly and socially isolated individuals, including the oldest old, family caregivers, former carers and the bereaved. It targets hidden caregivers and their families, those who are very lonely or at risk of becoming socially isolated or who have been overlooked by others, especially carers in crisis looking after someone with a long-term or complex life-limiting condition, or who are themselves facing the end of life.
